How to change link of list item's title in sharepoint custom list by pro-grammatically on SharePoint Hosted Apps(Java Script) ?

Can any one suggest me, How can I change link of list item'stitle to my custom page with parameter ID of link redirection on click of the item. noted I am using SharePoint Hosted SharePoint 2013 Apps model.
